Another 70 are reported to be wounded, most of them critically.
The bomb blast, the worst in Pakistan for two months, struck in Akhurwall village in the semi-tribal north-west area of Darra Adam Khel,140 kilometres west of the capital Islamabad.
"Several people are buried under the debris," said local administration official Gul Jamal Khan.
Ambulances and private volunteers rushed to bring the victims to hospital in the province’s main city, Peshawar.
Pakistani Taliban are reported to have claimed responsibility.
More than 3,770 people have been killed in suicide attacks and bombings, blamed on homegrown Taliban and other armed Islamist networks since government troops stormed a radical mosque in Islamabad three years ago.
